DOUGLAS

We can do 4 CE assessments per month as a favor to the Douglas County CoC. Complete the form for all clients.

Clients can be housed or homeless. They may or may not be referred to PCCI by the Douglas CoC. We have no control or knowledge of how long it takes from CE assessment to referral. All we can tell them is that Douglas CoC prioritizes clients who are in the most need based on their SPDAT score (part of the CE assessment).

Once the monthly quota of 4 CE assessments has been reached, there will be a notice in this box just below “DOUGLAS.”

GWINNETT

UPDATE, JUNE 1, 2025: We are not accepting any referrals for HUD Balance of State (Clayton, Henry, Rockdale, Gwinnett, Douglas*) Rapid Re-Housing projects at this time. Please refer clients to the appropriate Continuum of Care for their location.

We cannot accept self-referrals; all clients must enter the system through the Clayton CoC.

Fill out the form for all clients and refer them via the link below. We need the entire form filled out because there are occasions when we can accept self-referrals, but it’s random so it’s not fair to tell clients.
