If you are experiencing homelessness in Douglas County and are in need of assistance obtaining permanent housing, please complete this form and an Intake Coordinator will contact you shortly to complete an eligibility screening.

Veteran’s Homelessness Prevention Assistance

PCCI’s SSVF program provides assistance for veterans who are at imminent risk of homelessness.

We are at capacity for the month of April.

We will resume eligibility screening for our SSVF program during the last week of the month. If you wish to get on the waiting list, please fill out the form on this page and an Intake coordinator will contact you at the end of April.

ELIGIBILITY

In order to be eligible for rental assistance you must:

  • Have a recent Dispossessory Warrant from the court
  • Have a lease in your (the veteran applicant) name
  • Complete a Homelessness Prevention pre-screen with an Intake Coordinator and meet program qualifiers.
