Supportive Services for Veteran Families (SSVF)
A program to assist Veterans and their families that are experiencing homelessness to become rapidly re-housed and self-sufficient
* Veteran means a person who served in the active military, naval, or air force, and who was discharged or released there from under conditions other than dishonorable.
Documentation of Veteran status is required for SSVF enrollment.
PCCI Services cover participants experiencing homelessness in City of Atlanta and the following counties: Fulton, DeKalb, Clayton, Henry, Rockdale, Gwinnett, and Douglas (see map below).
Veterans are welcome to present at the Community Resource and Referral Cetner (CRRC) to determine eligibility for other agencies. The CRRC is located at:

AVAILABILITY
PCCI is conducting eligibility pre-screens for June SSVF assessments.
If you are currently sleeping on the streets, in an emergency shelter, or in a Grant-per-Diem (GPD) bed, complete the form and an Intake Coordinator will contact you for an eligibility screening.
If you are currently sleeping anywhere else, please use the Homelessness Prevention Form instead.

If you are a veteran currently experiencing homelessness, please complete this form and an Intake Coordinator will contact you to conduct in eligibility screening.
If you are with an agency and wish to refer a veteran for Rapid Re-Housing services, please use the Agency Referral page instead.
*Stays in institutional setting are dependent upon a number of factors. See the HUD website for details.
