Veteran’s Homelessness Prevention Assistance
PCCI’s SSVF program provides assistance for veterans who are at imminent risk of homelessness.
AVAILABILITY
PCCI is conducting eligibility pre-screens for June SSVF assessments.
If you are at risk of losing housing, read the “Eligibility” box to the right before completing the form on this page.
If you are temporarily staying with friends or family, couch-surfing, paying for a hotel, are in a recovery program not paid for by the VA, or are staying in an institution where you have been for 90 days, complete the form and an Intake Coordinator will contact you shortly to complete an SSVF eligibility screening.

ELIGIBILITY
In order to be eligible for rental assistance you must:
- Have a recent Dispossessory Warrant from the court
- Have a lease in your (the veteran applicant) name
- Complete a Homelessness Prevention pre-screen with an Intake Coordinator and meet program qualifiers.
