REQUEST FOR HOMELESSNESS PREVENTION ASSISTANCE
Priority for assessments goes to veterans who are currently unsheltered. We cannot guarantee that we can enroll everyone who requests assistance.
If you are with an agency and wish to refer a veteran for housing assistance, please fill out the form on THIS PAGE instead.
ELIGIBILITY
In order to be eligible for SSVF Homelessness Prevention assistance, you must:
- Be at imminent risk of homelessness within the next 30 days
- Have a current signed lease in your (the veteran applicant’s) name if you need assistance paying rent and/or arrears
- Complete a Homelessness Prevention pre-screen with an Intake Coordinator and meet program qualifiers
