This form is for agency referrals only. Please go to our Get Help page and click the “Get Info” button that corresponds to the county where you slept last night for information on how to enroll in that county’s Coordinated Entry system.
*If client’s situation does not match any of the selections, they are not eligible for services.
**If the client is staying in an institution, they must have entered less than 90 days ago and on the night immediately prior to entering that institution, they must have been staying in a place not meant for human habitation or an emergency shelter.
ELIGIBILITY
In order to be eligible for PCCI Rapid Re-Housing services, the client must be literally homeless at the time of referral. PCCI’s HUD RRH programs do not provide prevention services at this time.
