PCCI is always available for veterans in need of housing assistance.

If you are currently sleeping on the streets, in an emergency shelter, or in a Grant-per-Diem (GPD) Transitional Housing bed, complete the form and an Intake Coordinator will contact you for an eligibility screening.

For the month of July, 2025, we have a limited number of SSVF assessment appointments available. Priority for these appointments goes to veterans who are currently sleeping in places not meant for human habitation, such as outside, in a vehicle, bus shelter, or abandoned building.

If you are currently housed, staying temporarily with friends or family, couch-surfing, paying for a hotel, or in a treatment program, please use the Homelessness Prevention Form instead.
