PCCI has no more SSVF enrollment openings for the month of September, 2025. If you are currently unsheltered, in an emergency shelter, or in Grant-per-Diem (GPD) Transitional Housing, please complete the form on this page and an Intake Coordinator will contact you shortly to complete a program eligibility screening.

If you are currently housed, staying temporarily with friends or family, couch-surfing, paying for a hotel, or in a treatment program, please use the Homelessness Prevention Form instead.
